Adult Family Care Homes (AFCHs)

An Adult Family Care Home (AFCH) is a private residential setting designed to provide personal care services to individuals who require assistance with daily living. The owner or provider may reside in the home and deliver care for up to six residents, offering a personalized and intimate environment. Services offered are comparable to those provided in Assisted Living Facilities (ALFs), with an emphasis on
individualized, one-on-one care.
Residents live in a home-like setting, either in private or semi-private accommodations, depending on availability and affordability. AFCHs typically offer comprehensive services such as meals, laundry, housekeeping, and medication management. A caregiver is present on-site 24 hours a day to ensure consistent care and supervision.
AFCHs support a wide range of care needs, including assistance with ambulation, dressing, bathing, grooming, eating, and continence management. They are also equipped to care for individuals with varying stages of Alzheimer’s disease and other forms of dementia. Like ALFs, AFCHs conduct regular assessments to ensure residents receive care tailored to their evolving needs.

Admission and Ongoing Assessment
Before admission to an AFCH, a personalized assessment is completed by the resident’s primary care physician in coordination with the facility’s clinical staff and administration. This assessment serves as a medical order, enabling the facility to determine the appropriate level of care and to identify any specialized services required. These assessments are reviewed and updated regularly to ensure each
resident continues to receive care that aligns with their current condition and needs.
